At Northrise, incident reporting is one among other ways in which members of the University community can participate to promote a healthy, safe and nurturing environment.

This incident report form is available to anyone who would like to notify the University about the following:

  • Conduct Violation: Incidents of potential violations of Student Code of Conduct or other university policy by community members (staff, students or faculty)
  • Matter of Concern: Issue or Incident involving community members that exhibit behaviors that cause concern for the well-being of the community and/or its property
  • Academic Integrity Violation: Incidents involving potential violation of the Academic Integrity Policy
  • Accident Incidents: Accidents or near misses involving community and or property

If there is immediate danger and/or significant risk of health, or safety, please contact the Student Life Office or the Secuirty Team

Confidentiality Pledge: The University assures the highest confidentiality when dealing with reported incidents.
